Week Day - Mixed Billing Practice

As part of our philosophy to offer the highest standard of care to all members of the community,

Shelley Hub Family Practice is a Mixed billing on a weekday.

  • Consultations on a weekday are Mixed billed.
  • Children up to 16 years old are bulk billed.
  • Concession cardholders are bulk billed.

Appointments

Standard appointments
For consultations to discuss one or two issues.

Long appointments
Long consultations (used to discuss multiple issues over 20 minutes)

Immunisation
Please call us to make an appointment for the Doctor and the nurse.

Telehealth Appointments

Video or phone calls. Please use our AMS (booking system) to schedule an appointment or call our receptionist.

Only available for patients who have seen the doctor face-to-face in the past 12 months.

Walk-in Appointments

Walk in patients are triaged as appropriate. Non-urgent walk in patients are accepted
if there is an available appointment, or scheduled to when there is an appointment.

Missed appointments

We understand that things change, therefore, if you need to cancel or reschedule your appointment, please call us or manage your appointment via AMS - Connect.

Missed appointments or not cancelled 2 hours prior will attract fee of $50

How to provide Feedback

We are always striving to improve our standard of professional service for our patients. If you have suggestions on how we can make improvements or if you are unhappy with the service you have received, please speak to a staff member. Alternatively, we encourage you to write an email to the Practice Manager at [email protected].

If you feel that our practice has not properly dealt with your complaint, you may contact the Government of Western Australia - Health and Disability Services Complaints Office (HaDSCO) by telephone at 08 6551 7600 or via email at [email protected].


Home visits

If a patient's illness prevents them from coming to the clinic, they can request a home visit by speaking with the receptionist. The decision to conduct a home visit is at the discretion of the doctor, if doctor is not able to do Home Visit, alternative options will be explored and provided to the patient (such as phone/video consultation, Night Doctor home visit 1300 644 483)
